Documents Required

The required documents vary depending on the place of birth and the time of registration.

1. Institutional Birth (Hospital / Nursing Home)

When a birth occurs in a hospital, the hospital administration is legally bound to report the birth to the local registrar via the CRS portal.

  • Discharge Slip / Birth Proof / Hospital Receipt issued by the hospital.
  • Aadhaar Card of both parents.
  • Address Proof (Voter ID, Electricity Bill, etc.).
  • Note: The hospital will provide you with a Registration/Reference Number. You can use this to collect the final certificate from the local body (Nagar Nigam/Palika).

2. Non-Institutional Birth (Home Birth)

If the birth takes place at home, the head of the family must report it to the local registrar within 21 days.

  • Aadhaar Card of both parents.
  • ID Proof of the Informant (the person reporting the birth).
  • Certification letter from an Anganwadi worker, ASHA worker, or Village Head (Gram Pradhan).

How to Apply for a Birth Certificate

There are three primary ways to apply for a birth certificate in India:

Method 1: Via the CRS Portal (crsorgi.gov.in) for Public Users

If you are applying within 21 days of birth and your state allows direct public entry on the CRS portal:

  1. Visit crsorgi.gov.in and click on “General Public Signup.”
  2. Complete the registration using your mobile number, email, and captcha.
  3. Log in to your dashboard and click on “Add Birth Registration.”
  4. Fill in the child’s details, parents’ details, and permanent/current address.
  5. Upload the hospital receipt or required supporting documents.
  6. Submit the form and save the Reference Number.
  7. Print the submitted form and submit it physically to your local Registrar’s office (Nagar Nigam or Gram Panchayat) within 7 days.

Method 2: Via State e-District Portals (For CSC VLEs & Citizens)

In many states (such as Uttar Pradesh, Madhya Pradesh, Rajasthan, and Bihar), direct public entry on the central CRS portal is restricted. Applications must go through the state’s e-District portal.

  • For CSC VLEs: Log in to the Digital Seva Portal -> Search for e-District in Services -> Select Birth Certificate Service -> Fill out the citizen’s form and process the payment via the CSC Wallet.
  • For Citizens: Create a “Citizen Login” on your respective state’s e-District portal (e.g., edistrict.up.gov.in) and fill out the online application.

Method 3: Offline Application (Gram Panchayat / Municipal Corporation)

If portals are down or you are applying for a delayed registration (>1 year):

  1. Obtain the physical application form from your Municipal Corporation or Gram Panchayat office.
  2. Fill it out and attach the hospital receipt, parents’ IDs, and affidavits (if applicable).
  3. Submit the file to the registrar and collect your Acknowledgement Slip.

Child Name Addition: How to Add a Name to the Certificate

It is common for a child not to be named immediately at birth. In such cases, the hospital issues a certificate under the title “Baby of [Mother’s Name]”.

  • The Rule: As per Section 14 of the RBD Act, a child’s name can be added to the registry free of cost within 1 year of the registration date.
  • After 1 Year: The name can still be added up to 15 years later, subject to a late fee. Adding a name after 15 years requires a much more complex legal procedure.
  • The Process: You can apply online via crsorgi.gov.in or submit a physical “Name Addition Form” at your local registrar’s office along with the parents’ Aadhaar cards.

Correction / Update: How to Fix Mistakes

If there is a clerical error (spelling mistake in names, incorrect date of birth, or address mismatch) in the issued certificate, it can be corrected under Section 15 of the Act.

  1. Application: Corrections can usually only be processed by the specific local registrar office that issued the original certificate.
  2. Documents: You will need the parents’ correct Aadhaar cards, 10th-grade mark sheets (if applicable), and a Notarized Affidavit explicitly stating the error and the correct information.
  3. Process: The registrar will verify the affidavit, cancel/amend the old entry, and issue a fresh certificate. Note: This process is for correcting spelling errors only, not for a complete legal Name Change.

How to Download Your Birth Certificate (DigiLocker)

Once your certificate is approved and generated, you can easily download a digitally signed copy:

Via DigiLocker (Recommended & Easiest Method):

  1. Download the DigiLocker App or log in to the web portal.
  2. Navigate to “Search Documents” and type “Birth Certificate.”
  3. Select your state’s issuing department (e.g., Municipal Corporation / e-District).
  4. Enter your Application Number / Registration Number.
  5. Your digitally signed Birth Certificate will be fetched and saved to your account. This digital copy is 100% legally valid everywhere.

Via CRS Portal: If you applied as a public user on the CRS portal, simply log in to your dashboard and select the ‘Print Certificate’ option next to your approved application.

Common Problems and Solutions

  • Problem: The child is 5 years old and doesn’t have a certificate. How to apply?
    • Solution: You must undergo the Delayed Registration process. You will need a Non-Availability Certificate (NAC) and an SDM (Magistrate) order. It is best to consult a local CSC center or a lawyer to draft the necessary affidavits and file for the SDM order.
  • Problem: The hospital says they uploaded the details online, but I haven’t received the certificate.
    • Solution: Ask the hospital for the CRS Registration/Reference Number. Take this number to your local Municipal Corporation (Zonal Office) to get the certificate printed.
  • Problem: Payment failed on the portal, but money was deducted.
    • Solution: Wait 24 to 48 hours. If the status remains “Failed,” the amount will automatically be refunded to your bank account. Do not pay again until the first transaction is clearly marked as failed.

Services for VLEs / CSC Operators

A universal CSC/VLE login or registration facility for birth certificates is not clearly specified on the official CRS source. Operator access, if available, depends on the relevant state or local authority.

Help & Grievance

For registration issues, contact the concerned Registrar of Births and Deaths or the relevant municipal, panchayat, or state registration authority. The official CRS website provides departmental information and guidance.
