Death Certificate – Services, Apply, Track & Official Links

A Death Certificate is a highly sensitive and crucial legal document issued by the government to officially record a person’s demise. In India, under the Registration of Births and Deaths (RBD) Act, 1969, it is mandatory to register every death with the local authorities.

Following the loss of a family member, this certificate is the primary document required to settle legal and financial matters. It is mandatory for transferring property (Mutation), claiming Life Insurance, closing bank accounts, and accessing pension benefits.

Whether you are a citizen looking for the correct procedure or a CSC VLE assisting families, this comprehensive guide covers everything from application processes and mandatory documents to Delayed Registration rules, correction procedures, and digital downloads.

The CRS portal supports reporting eligible death events and obtaining certificates. Registration is handled by the competent local Registrar of Births and Deaths.

Documents Required

The documents required to obtain a death certificate vary based on where and how the death occurred:

1. Natural Death in a Hospital (Institutional Death)

When a death occurs in a hospital or nursing home, the administration is legally bound to report it to the local registrar.

  • Death Summary / Discharge Ticket issued by the hospital.
  • MCCD (Form 4): Medical Certificate of Cause of Death (Provided by the attending doctor).
  • Aadhaar Card or Any ID of the deceased.
  • ID Proof of the Applicant (Informant / Family Member).

2. Natural Death at Home (Non-Institutional Death)

If the passing occurs at home, the head of the family must report it to the local registrar within 21 days.

  • Aadhaar Card / Voter ID / PAN Card of the deceased.
  • ID Proof of the Informant.
  • Cremation Ground / Burial Receipt: This is the most critical proof of death.
  • MCCD (Form 4A): If the deceased was undergoing treatment at home under a medical practitioner.
  • Certification letter from a local Ward Member, Gram Pradhan, or ASHA worker.

3. Unnatural Death (Accident, Suicide, or Homicide)

In medico-legal cases, the process involves law enforcement, and standard hospital receipts are not enough.

  • FIR (First Information Report) copy.
  • Post-mortem Report copy.
  • Police Inquest Report (Panchnama).
  • Cremation / Burial Receipt.
  • IDs of the deceased and the applicant.

4. Delayed Registration (After 1 Year)

  • Court Order issued by the SDM / Magistrate.
  • Non-Availability Certificate (NAC): Proof from the local registrar that the death was never registered previously.
  • Notarized Affidavit from the applicant explaining the delay.

How to Apply for a Death Certificate

There are three primary methods to apply for a death certificate in India:

Method 1: Via the CRS Portal (crsorgi.gov.in) for Public Users

If you are applying within 21 days of death and your state allows direct public entry on the CRS portal:

  1. Visit crsorgi.gov.in and click on “General Public Signup.”
  2. Complete the registration using your mobile number and email.
  3. Log in to your dashboard and click on “Add Death Registration.”
  4. Fill in the deceased’s details, place of death, and the informant’s details.
  5. Upload the cremation receipt or hospital documents.
  6. Submit the form and save the Reference Number.
  7. Print the submitted form and submit it physically to your local Registrar’s office (Nagar Nigam or Gram Panchayat) within 7 days.

Method 2: Via State e-District Portals (For CSC VLEs & Citizens)

Many states (like Uttar Pradesh, Madhya Pradesh, Rajasthan, and Bihar) restrict direct public data entry on the central CRS portal. Applications must go through the state’s e-District portal.

  • For CSC VLEs: Log in to the Digital Seva Portal -> Search for e-District in Services -> Select Death Certificate Service -> Fill out the form and pay via the CSC Wallet.
  • For Citizens: Create a “Citizen Login” on your state’s e-District portal (e.g., edistrict.up.gov.in) and fill out the online application.

Method 3: Offline Application (Gram Panchayat / Municipal Corporation)

If the portals are down or you are applying for a delayed registration:

  1. Obtain the physical Death Registration application (Form 2) from the Municipal Corporation, Tehsil, or Gram Panchayat.
  2. Fill it out and attach all required documents (Cremation receipt, IDs, affidavits).
  3. Submit the file to the registrar and collect your Acknowledgement Slip.

Delayed Registration: How to Apply After 1 Year

If more than 12 months have passed since the death, the local registrar loses the authority to issue a certificate directly. You must follow the legal SDM route:

  1. Apply to your local registrar for a Non-Availability Certificate (NAC). This proves no prior certificate exists.
  2. Hire a local advocate to file an application for Delayed Registration in the Sub-Divisional Magistrate (SDM) or First-Class Magistrate’s court.
  3. Submit an Affidavit explaining why the registration was not done earlier.
  4. The SDM will order an inquiry (usually conducted by local police or a Lekhpal/Patwari) to verify the facts.
  5. Once verified, the SDM will pass a Court Order. Submit this order to your local registrar, who will then generate the Death Certificate.

Correction / Update: How to Fix Mistakes

If there is a clerical error (e.g., spelling mistake in the deceased’s name, incorrect date of death, or parent/spouse name mismatch), it can be corrected under Section 15 of the RBD Act.

  1. Application: Corrections can only be processed by the exact local registrar office that issued the original certificate.
  2. Documents: You must provide the original hospital records, the deceased’s correct government IDs, and a Notarized Affidavit explicitly stating the error and the correct information.
  3. Process: The registrar will verify the evidence, cancel/amend the old entry in the registry, and issue a fresh, corrected certificate.

Services for VLEs / CSC Operators

A universal CSC/VLE login or registration facility for death certificates is not clearly specified on the official CRS source. Operator access, if available, depends on the relevant state or local authority.
