How to Apply: Step-by-Step UDID Card Process
It is crucial to understand that simply filling out the online form does not generate a certificate. A physical medical examination is mandatory.
Step 1: Online Registration
- Visit the official portal swavlambancard.gov.in.
- Click on “Apply for Disability Certificate & UDID Card.”
- The form is divided into four sections:
- Personal Details: Name, parents’ names, DOB, blood group.
- Disability Details: Type of disability, since when (birth or later), and if you have an old certificate.
- Employment Details: Current employment and income status.
- Identity Details: Aadhaar number and captcha.
- Upload your Photo, Signature, and Aadhaar Card.
- Select your nearest District Hospital (CMO Office) where you wish to undergo the medical examination.
- Submit the form. You will receive an Enrollment Number. Print the application form and the receipt.

Temporary vs. Permanent Disability Certificate
Based on the medical board’s assessment, your certificate will fall into one of two categories:
- Permanent: If the disability is irreversible (e.g., amputation or blindness), the certificate is issued for a lifetime.

How to Download e-UDID / Disability Certificate
Once your card is approved, you don’t need to wait for the postman:
- Go to the portal and click on “Download your e-Disability Card & e-UDID Card.”
- Log in using your Enrollment Number and Date of Birth.
- On the dashboard, you will see two options:
- Download Disability Certificate (PDF)
- Download e-UDID Card (PDF)
- Download and print both. These digital printouts are 100% legally valid everywhere.
Common Problems and Solutions
- Problem: It has been 2 months since I applied online, but the status still says ‘Submitted.’
- Solution: You have not completed the mandatory physical medical assessment. An online application will not move forward without a doctor’s report. Take your application printout to the district hospital’s medical board immediately.
- Problem: I lost my Enrollment Number.
- Solution: You can still track your status or download your e-UDID by logging into the portal using your Aadhaar Number instead of the Enrollment Number.
- Problem: I already have an old offline (paper) certificate. Do I need to undergo a medical test again?
- Solution: When applying on the portal, select “Already have Disability Certificate” and upload a scanned copy of your old certificate. In many cases, the CMO can verify the old data and issue a UDID without requiring a new medical test (though this is at the CMO’s discretion).
